Solar Feed In

I have purchased a 5 year old house that had solar installed during the build process. Unfortunately the solar feed in was never activated on the smart meter, I have contact AGL about getting this activated however they said that I need a number of forms the installer. The previous owner doesn't have the forms and the builder isn't been very helpful.

How would I get the feed in feature activate if I don't have the required forms?
